Beim Plattfu\u00df z.B. kommt es zur Abfla\u00adchung des Fu\u00dfgew\u00f6lbes, die wiederum oft mit einem Knick\u00adsenkfu\u00df kombiniert ist.<\/p>\n<p>Als Folge f\u00e4llt das gesamte Fu\u00dfskelett zusammen, was zu einer Verminderung der Statik und Belastbarkeit des Fu\u00dfes so\u00adwie im weiteren Verlauf zur Arthrosenbildung f\u00fchrt.<\/p>\n<p>Der Tibialis posterior-Muskel (TP) ist der wichtigste Muskel f\u00fcr die Einw\u00e4rtsdrehung und die Statik des Fu\u00dfes. Er entspringt an der R\u00fcckseite des Unterschenkels und verl\u00e4uft hinter dem Innenkn\u00f6chel vorbei zum \u00dcbergang zwischen R\u00fcckfu\u00df und Fu\u00df\u00adwurzel, wo er f\u00e4cherf\u00f6rmig ansetzt.<\/p>\n<p>Bei einer strukturellen Schw\u00e4che oder Defekt der Sehne kann der Fu\u00df nicht mehr oder schlecht gegen Widerstand einw\u00e4rts gedreht werden. In der Folgezeit kommt es dann zum Einbruch des Fu\u00dfl\u00e4ngsgew\u00f6lbes mit allen Fehlbelastungsfolgen, wie zum Beispiel Arthrosen am R\u00fcckfu\u00df.<\/p>\n<p>Therapeutisch werden Fu\u00dfdeformit\u00e4t im Anfangsstadium mit einer orthop\u00e4dischen Fu\u00dfbettung, d.h. mit ma\u00dfgefertigten Einla\u00adgen, medial aktivierend, einer Physiotherapie (z.B. mit Spiraldy\u00adnamik) und\/oder entz\u00fcndungshemmenden Medikamenten an\u00adgegangen. Weitere ma\u00dfnehman sind dann Schuhzurichtungen bis zur verordnung und Anpassung von orthop\u00e4dischen Schuhwerk.<\/p>\n<p>Bei Versagen dieser konservativen Ma\u00dfnahmen oder schwerer Fehlstellung ist ein operatives Vorgehen zur Wiederherstellung des Fu\u00dfskelttes gegeben. Je nach Ursache wird an den Weichteilen (B\u00e4nder, Sehnen, Gelenkkapsel) und\/ oder auf Knochenebene (Osteotomien oder Versteifung von R\u00fcckfu\u00dfgelenken) korrigiert.<\/p>\n<\/div><\/section><\/div><\/div><!--close column table wrapper. Neben einer ausf\u00fchrlichen und Anamnese sind zur exakten operativen Planung standardisierte R\u00f6ntgenaufnahmen notwendig.<\/p>\n<p>In den ersten Tag hat die Schonung des operierten Bereichs oberste Priorit\u00e4t. Neben dem Hochlagern der Extremit\u00e4t geh\u00f6ren auch k\u00fchlende Umschl\u00e4ge (Eisbeutel), wenn m\u00f6glich mehrfach am Tag f\u00fcr jeweils zehn Minuten anwenden, zum postoperativen Behandlungsprogramm.<\/p>\n<p>Standardisiert werden am ersten, vierten und achten postoperativen Tag der Verband gewechselt und am zw\u00f6lften Tag die F\u00e4den gezogen. Zus\u00e4tzliche Verbandswechsel werden nach Bedarf durchgef\u00fchrt. Begleitet wird der station\u00e4re Aufenthalt ebenfalls durch die Physiotherapie. So erhalten Sie in den meisten F\u00e4llen w\u00e4hrend der Operation einmalig eine Antibiotika-Infusion. Im Anschluss an die Operation wird eine ausreichende Schmerzmedikation (Analgesie) festgelegt und in der Regel durch die \u00c4rzte der An\u00e4sthesie verabreicht. Sollte dies nicht ausreichen, erhalten Sie weitere Schmerzmedikamente. In den ersten Tagen nach der Operation sind zus\u00e4tzliche abschwellende und schmerzstillende Medikamente (NSAR) notwendig. Achten Sie hier auf die Vertr\u00e4glichkeit und teilen Sie dies bei Problemen unserem Personal mit. Zus\u00e4tzlich ist in der Anfangsphase eine Thromboseprophylaxe durch so genannte Heparinspritzen einmal t\u00e4glich notwendig. Die Dauer der Spritzen wird individuell festgelegt. Bei der Nachbehandlung mit einem Walker sind die Injektionen f\u00fcr den gesamten Zeitraum notwendig.<\/p>\n<p>Am Entlassungstag erhalten Sie neben dem Entlassungsbrief f\u00fcr den Hausarzt oder den weiterbehandelnden Facharzt auch eine \u00dcbungsanleitung unserer Physiotherapie zum weiteren selbst\u00e4ndigen Be\u00fcben des operierten Fu\u00dfes. Des Weiteren vereinbaren Sie bitte in der Praxis einen Termin zum F\u00e4den ziehen sowie den 4 Wochen bzw. 6 Wochen Post-OP-Kontrolltermin.<\/p>\n<p>Auch zu Hause sollten Sie den operierten Bereich weiter schonen, k\u00fchlen und hochlagern.
